To get Blocked Number for chapter 5, you will need the bar to favor Kotonoha following the trip to the pool. This is generally the easiest route to follow, as the bar will already favor Kotonoha coming from The One I Truly Love.
If for any reason the bar favors Sekai following the pool trip, you will get either Left Behind or Getting Over It instead.
The Blocked Number route effectively merges into Kotonoha Abandoned.
